About Diabetes
Diabetes is a disease that results in high glucose (sugar) levels in a person’s blood, resulting from inadequate levels of insulin, or resistance to insulin action. It is the sixth leading cause of death in New Jersey among adults. Untreated, it can lead to complications such as kidney disease, heart disease, stroke and even blindness. Even if you don’t have diabetes, your doctor can screen and identify prediabetes (meaning at high risk for developing diabetes) and take action to prevent diabetes. If you are living with diabetes, BHealthyCare has the resources to support you!
Diabetes Management Program
Employees and their covered family members taking diabetes medication are eligible for the Diabetes Management Program, which eliminates copays for those medications.
Program Requirements
To continue to receive the copay waiver for 2025, members must complete an annual wellness visit with their Primary/Family Care Provider (PCP) by October 31, 2024.
Anyone who is taking a diabetes medication and had a wellness visit with their PCP from November 1, 2023 – October 31, 2024 will continue $0 cost share for 2025.
Why do I have to see my PCP?
Your PCP is a key member of your care team who can help you manage other health concerns along with your diabetes. In addition to your annual wellness visit, you may be due for a retinal screening (eye exam), diabetic foot exam, or other preventive screenings.
Insulin and Supplies
Getting your annual wellness visit will ensure $0 cost share for diabetes medications in 2025 – but rest assured that insulin and supplies will automatically be covered 100% for you with $0 cost share.
Getting started
The BHealthy Care Management team can assist you with making an appointment with your PCP, discuss your current treatment plan, and connect you with other resources to help you manage your diabetes. The team is available Monday through Friday from 9am to 5pm at 844.424.2628, Option 5.
