What is diabetes?

Are you living with diabetes?

Know that you're not alone. As a member of the RWJBarnabas family, we're here to support you.
What is Diabetes?

Diabetes is a disease that results in high glucose (sugar) levels in a person’s blood, which prevents the body from using insulin properly. Diabetes is the sixth leading cause of death in New Jersey among adults. Even if you don’t have diabetes, your doctor may see some signs that show you could develop it; this is prediabetes. The good news is that you can take steps now to prevent it.

How can RWJBarnabas help me manage my condition?

The medical plan provides comprehensive coverage for diabetes management. It is critical to get the proper medical treatment to avoid a serious health event.

How do I find a doctor to manage my diabetes?

People with diabetes who are treated with insulin shots generally should see their doctor at least every three to four months. People with diabetes who are treated with pills or who are managing diabetes through diet should be seen at least every four to six months. More frequent visits may be necessary if your blood sugar is not controlled or if complications of diabetes are worsening.

You can locate an RWJBarnabas Health Medical Group provider using Horizon’s provider lookup tool. If you need assistance, you can also contact Health Advocate.

Lab Tests

It is important to go for lab tests based on your doctor’s recommendations; such as fasting plasma glucose tests, oral glucose tolerance tests, and HbA1c. You will pay less ($0 if enrolled in the OMNIA plan) by going to RWJBarnabas Health labs for your tests.

Insulin Pumps & Digital Monitoring Devices

It is best to utilize the medical plan (rather than your pharmacy benefit) to obtain your insulin pump.  If you need assistance with obtaining an insulin pump, contact Health Advocate for support.

Other supplies

LSC Pharmacy provides discounted pricing for insulin and diabetic supplies. Therefore, insulin and glucose testing supplies can only be obtained through the LSC Mail Order Pharmacy. Please note:

  • Trividia’s True Metrix and Arkray’s Glucocard are preferred glucose testing supplies at $0 copay.
Other glucose testing supplies have copays.

  • If a member is an insulin pump utilizer, and the pump requires specific glucose testing supplies, the copay is $0.
